STATEMENT OF PURPOSE

The Clinical Care Coordinator plans, directs and participates in direct and indirect patient care and other unit activities in order to provide quality patient care.

JOB RESPONSIBILITIES

  • Demonstrates an ongoing knowledge of the overall activities, patient case load and acuity within the department, giving clinical assistance as needed.
  • Plans safe and efficient nursing care and implements the policies and procedures of the hospital, the department and Patient Care Services.
  • Ensures that care is delivered to each patient in accordance with quality standards of care and documented in the clinical record.
  • Delegates responsibilities within the scope of staff abilities to provide continuity of care.
  • Continuously assesses and evaluates nursing care given within the department and communicates findings to Director.
  • Promotes an environment in which the patient care team can work cooperatively toward objectives and strive for excellence in patient care.
  • Demonstrates and promotes good interpersonal relationships and positive staff morale within the department.
  • Assists Director with formation and implementation of policies and procedures.
  • Confers and works closely with the Director regarding staff changes, needs and problems, seeking advice when needed.
  • Monitors clinical record to assist in resolving patient problems in a timely manner.
  • Works closely with the Director to assure appropriateness of ongoing QI monitoring on the unit and in preparation for QI reports.
  • Has attended a minimum of one Advanced Fetal Monitoring course.
  • Demonstrates the ability to initiate and direct life support measures by performing effectively in Code Blue, Code Pink and Code Red situations.
  • Demonstrates basic knowledge of the standards of practice of AWHONN guidelines.
  • Demonstrates knowledge of breastfeeding instruction and techniques.
  • Contributes to monthly staff meetings.
  • Assists the staff in monitoring supplies and equipment usage being alert for and taking measure to control waste
  • Contributes pertinent observations to employee performance improvement process.
  • Maintains a current understanding of policies/procedures and memoranda of the hospital and interprets same to staff and when necessary, to physicians.
  • Demonstrates a working knowledge of the legal responsibilities of the hospital to the patient, physician, and nursing personnel and when in doubt, confers with the Director.
  • Demonstrates a complete knowledge of location, care and operation of all equipment for area of responsibility.
  • Demonstrates the ability to effectively perform in the role of circulating nurse/scrub nurse and the recovery of c-section patients.
  • Assist the Director in maintaining all statistical data on a monthly basis
  • Works with the Director to maintain an optimum staff of qualified/competent personnel on duty by working with the department schedule
  • Works with the clinical director to ensure that appropriately trained/oriented staff is assigned according to patient needs and that all assignments are based on acuity.
  • Works with the clinical director to maintain all changes on the employee time schedule keeping overtime to a minimum and ensuring safe staffing levels
  • Assumes direct care and responsibilities as appropriate
  • Demonstrates the ability to assess, treat, and care for pediatric, adolescent, adult and geriatric patients.

    QUALIFICATIONS

  • Must be a graduate of a school of nursing.
  • Must hold current licensure as a Registered Nurse in the State of Georgia.
  • Minimum of two (2) years clinical experience with charge duties preferred.
  • BLS/NRP certification required. ACLS/NRP certification preferred or willing to obtain.
