JOB SUMMARY

This position is responsible for assuring compliant implementation of the Quality Improvement Program for Newton Medical Home Health. Maintains knowledge of new or changed regulations/requirements and facilitates implementation of new or changed regulations/policies, monitoring, problem identification, education, and reporting outcomes and recommendations.

This position also admits patients; provides skilled nursing care as outlined in the nursing care plan and guided by the Nurse Practice Act and other state and federal regulations; documents the care given; and assists with the coordination of patient care. These services are performed in accordance with physician orders as coordinated by the Case Manager under the direction and supervision of the Clinical Supervisor.

DUTIES AND RESPONSIBILITIES

  • Completes an average of 2.5 visits per day (or the equivalent) when they are available.
  • Stays abreast of legislative regulations (State/M'Care/Other), JCAHO requirements, CLIA
  • Provides input into ongoing structure of QI Program
  • Implements the QI process to assure agency is in compliance: Assessment, Problem Identification, Education, Monitoring, Follow-Up, Resolution and/or Redirecting Efforts
  • Assists Managers/Administrative staff in identifying providers with documentation deficiencies and Plans of Correction
  • Arranges in-service education as needed to inform and correct concerns.
  • Assist in orientation for new staff
  • Functions as a liaison with other Newton Medical Managers to assure an integrated QI program with Newton Medical Center.
  • Participates on a rotating basis in 24 hour office coverage primarily for patient emergency and questions.
  • Attends departmental staff meetings, case conferences, and in-services as scheduled.
  • Actively participates on Quality Improvement Program Committee.
  • Functions as agency Clinical Supervisor in the event of their absence.
  • Actively participates in the UR/QI Chart Audit.

QUALIFICATIONS

Education and Training: Graduate of an accredited school of nursing; Current Licensure as a Registered Nurse in the State of Georgia

Experience: At lease 2 years of previous Home Health experience required.
